M/A-COM is a part of the Wireless Systems Segment of Tyco Electronics and offers an array of benefits-from health insurance and income protection programs to "time away from work" and educational assistance.

This overview includes information about many of the benefit programs available to our employees.

Health Care
Tyco Electronics is committed to ensuring that all employees have access to quality, comprehensive and affordable health care. The Tyco Electroncis Health Care Connection offers one medical plan - either a Point-of-Service (POS) Plan or a Preferred Provider Organization (PPO) Plan - depending on an employee's address. If the employee does not live in a POS or PPO network area, the employee is eligible for coverage under the Tyco Electronics Traditional Health Plan..

Point-of-Service (POS) Plan
In-Network

  • Choose a primary care physician (PCP) to manage all care.
  • Office visits are covered at 100% after a $15 co-pay per visit.
  • Most inpatient and outpatient care is covered at 80% up to out-of-pocket maximum; at 100% thereafter. There is a deductible.
  • Annual out-of-pocket costs are capped at $1,750 per person, $3,500 per family.

Out-of-Network

  • Employee can elect to go outside the network for care, but coverage will be less than staying in-network.
  • Pay a calendar-year deductible of $400 per person before the plan pays benefits
  • After paying a deductible, inpatient care is covered at 65% up to your out-of-pocket maximum; at 100% thereafter.
  • Office visits are covered at 65%, after deductible.
  • Annual out-of-pocket costs are capped at $4,000 per person.

Preferred Provider Organization (PPO) Plan:

  • This plan is similar to the POS Plan, with the following exceptions:
  • Receive care from a PPO network provider for in-network coverage levels (described under POS plan).
  • Receive care from non-PPO network providers for out-of-network coverage (described under POS plan).
  • There is no primary care physician designation required, but you are encouraged to use one doctor to manage your care


Tyco Traditional Health Plan

  • Receive care from any provider.
    Pay a calendar-year deductible of $300 per person, $600 per family, before the plan pays benefits.
  • After deductible, most inpatient care is covered at 75% up to out-of-pocket maximum; at 100% thereafter.
  • Office visits are covered at 75%, after deductible.
  • Annual out-of-pocket costs are capped at $2,500 per person, $5,000 per family.
  • Have the option of enrolling in a POS or PPO plan for a higher level of coverage, if willing to travel to network area

Prescription Drug Program
To help manage the cost of prescription drugs, Tyco Electronics includes the Prescription Drug Program when an employee elects coverage under a Tyco Electronics medical plan. Prescriptions may be filled at a participating pharmacy, paying only a co-payment, with no deductible required.

At a participating pharmacy, employee pays a co-payment of:

  • $10 for generic drugs
  • $20 for preferred brand-name drugs for up to a 30-day supply of drugs

A convenient mail order program is also available for drugs.

For a 90-day supply of mail order drugs, you'll pay a co-payment of:

  • $15 for generic drugs
  • $ 30 for preferred brand-name drugs

Employees may choose non-preferred brand-name drugs, paying $40 for up to a 30-day supply or $60 for a 90-day supply through the mail order program. In most instances, non-preferred brand-name drugs have either a generic or preferred brand equivalent.

Dental Plan
Maintaining good dental care is essential to good health. When you join Tyco Electronics, you and your eligible family members can join the Dental Plan.

Highlights of coverage:

  • Preventive care
    100%
  • Orthodontia covered at 50% for dependent children under age 19, up to a separate $1,500 lifetime maximum

After you pay a $50/person/calendar-year deductible

  • Basic Treatment
    80%
  • Major Treatment
    50%

Reimbursement Accounts
Employees can participate in a Health Care and/or Dependent Care Reimbursement Account to save income taxes on eligible health and dependent care expenses.

  • Employees can set aside up to $2,400 a year to pay for most unreimbursed health care expenses; and
  • Employees can set aside up to $5,000 a year ($2,500 if tax status is married, filing separately) to pay for eligible dependent day care.

Life Insurance
Everyone has different needs when it comes to life insurance. Tyco Electronics provides a variety of coverages for survivor protection. In addition to the basic coverages provided at no cost, employees may also buy additional coverage for themselves and their families.

Basic Term Life Insurance
A benefit equal to 1x your base annual salary is provided at no cost to the employee.

Accidental Death and Dismemberment Insurance (AD&D)
A benefit equal to 1x base annual salary is provided at no cost to the employee.

Business Travel Accident Insurance
This plan pays a benefit, at no cost to the employee, based on salary should the employee die or be injured while traveling on company business.

Personal and Family Accident Insurance
Employees may buy additional accidental death and dismemberment (AD&D) insurance for either the employee only or the employee and family. This plan pays a benefit in the event of a covered accident resulting in injury or death.

Supplemental Life Insurance
Employees may also buy additional life insurance at competitive group rates.

  • Life insurance for self: 1x, 2x, 3x, 4x, or 5x salary, up to $3,000,000.
  • Additional AD&D coverage to match the additional life insurance.
  • Spouse life insurance: from $10,000 to a maximum of $100,000.
  • Dependent child life insurance: $5,000 or $10,000 per child.

Disability

Short Term Disability
Minimum of 75% salary continues for up to 26 weeks if absent from work due to an illness, injury, or maternity. This benefit is provided at no cost to the employee.

Long Term Disability
All newly eligible employees may join the Long Term Disability Plan. This plan will cover 60% of pay should an employee become permanently and totally disabled. Coverage under this plan begins for employees who are totally disabled for more than 180 consecutive days.

Savings Program

401(k) Retirement Savings and Investment Plan
The Retirement Savings and Investment Plan is one of the best ways to save for retirement. Employees can enjoy the benefits of saving with pretax dollars, plus all earnings accumulate on a tax-deferred basis. In addition, employees may borrow from their savings while employed with Tyco Electronics.

Tyco Electronics will contribute to savings up to plan limits. A variety of investment options is available, including company stock.

The Tyco Electronics Retirement Savings and Investment Plan allows employees to directly roll over any balances from previous employers' 401(k) plans.

Employee Stock Purchase Plan
We put our stock in the future - and in our employees. Employees are immediately eligible to join the Tyco Electronics Employee Stock Purchase Plan (ESPP) upon hire. Tyco Electronics will make a 15% match on an employee's contribution and pays the brokerage fees for stock purchased under the ESPP.
