MAAM-011101 Ultra Small, Broad Band, General Purpose Amplifier

This device operates from 4 to 20 GHz, has typical Gain of 16 dB with +18 dBm of P1dB performance and is packaged in a convenient plastic 1.5mm by 1.2mm TDFN.
